Do you love helping people and making a real difference?

We’re on the lookout for a motivated and energetic person to lead a group of Support Workers and provide a short-term Reablement service. This role is perfect for someone who wants to promote independence and support people in their own homes.

What You’ll Do:

  • Manage and inspire a team of Support Workers.
  • Arrange rosters and cover for emergencies and leave.
  • Manage service referrals ensuring high-quality service delivery.
  • Support and develop your team through training and performance reviews.
  • Visit individuals to discuss their goals and how they can be achieved.
  • Continually monitor and improve service effectiveness.
  • Assess and recommend equipment to support daily living activities.

What We’re Looking For:

  • Experience in managing remote-based workers and providing mentoring and coaching.
  • Experience in a health and social care setting.
  • Awareness of the needs of individuals requiring reablement services and how to maximise their independence.
  • Strong organisational and computer skills.
